I understand that it is my responsibility and not that of Williamsburg Veterinary Clinic which includes all Veterinarians and staff, to have my pet's travel paperwork processed appropriately and in a timely manner after my pet's travel health exam.
I also understand that I will not hold Williamsburg Veterinary Clinic or it's employees accountable for any problems or difficulties processing said paperwork that arise may from a processing, shipping or other situations.
By signing, I am also accepting that the cost to perform and process an international health exam and certification is $679.98 (per pet, per country). This cost includes, the physical exam and consultation, USDA APHIS electronic submission fee, endorsement and processing costs, review of medical history and requirements for travel, filling out required documents and overnight shipping costs. If your pet does not need an endorsed USDA certificate and qualifies for a 7001 certificate, costs begin at $198.72 for exam and the certificate. Other fees may incur depending on the destination and their requirements for travel i.e. , FAVN (Rabies Antibody Test) microchipping, vaccines etc. Should any additional vaccinations, microchip placement or lab tests be required prior to my pet traveling overseas, I will be responsible for additional fees.
