I understand that it is my responsibility and not that of Williamsburg Veterinary Clinic which includes all Veterinarians and staff, to have my pet's travel paperwork processed appropriately and in a timely manner after my pet's travel health exam.
I also understand that I will not hold Williamsburg Veterinary Clinic or it's employees accountable for any problems or difficulties processing said paperwork that arise may from a processing, shipping or other situations.
By signing, I am also accepting that the cost to perform and process an international health exam and certification is $417.33 (per pet, per country). If your pet does not need an endorsed USDA certificate and qualifies for a 7001 certificate, costs begin at $184 for exam and the certificate. Other fees may incur depending on the destination and their requirements for travel i.e. , FAVN (Rabies Antibody Test) microchipping, vaccines etc. Should any additional vaccinations, microchip placement or lab tests be required prior to my pet traveling overseas, I will be responsible for additional fees.
